Detailed Criteria
for Admissions
|
- All persons accepted for service whose care
requires medical orders shall be under the care
of a physician, or other licensed practitioner
within his/her scope of practice.
- Require intermittent skilled care, intravenous
therapies, enteral therapies, and medication regimens.
- Have a diagnosis and therapy appropriate for
home care.
- can benefit
by skilled services or teaching
- have a condition
that will be stable or at least predictable.
- Is home bound according to Medicare Guidelines.
Have Physician's order/ Plan of Care and services
follow the written plan which is reviewed periodically
by a doctor. (484.18)
- Have source of payment or be willing to make
financial arrangements over time.
- Require services that can be adequately provided
by Haven HealthCare™ in the patient's home.
- Reside in geographical area serviced by Haven
HealthCare™.
- Haven
HealthCare™ will accept patients for
nursing services that are located within 2
hours driving time of our office in Signal
Hill, California.
- if the
patient lives outside of the acceptable area,
we will recommend nursing to be provided by
a home health agency located near the patient.
- Require nursing services offered by Haven HealthCare™.
- Haven HealthCare™
will only provide infusion and enteral therapies
that have been approved by the agency and
for which appropriate policies and procedures
have been written.
- Haven
HealthCare™ provides the following infusion
therapies to patients (via contracted services):
- antibiotic & anti-infective drugs
(IV)
- total and partial Parenteral nutrition
(IV)
- pain management (IV, SQ, Epidural, Intrathecal)
- chemotherapy (IM, IV)
- anticoagulants (IV, SQ)
- anti-emetics (PO, IV, IM, Pr)
- IVIG (IV)
- prolastin (IV)
- enteral therapy
- steroids
- colony stimulating factors
- hydration
