Who
can get access to home health care?
- Anyone who is essentially
homebound:
- Recovering from an illness
- Recently had a change in
their medical regimen
- In need of therapy for rehabilitation
How can I access home
health care?
There are several ways that
the process can be initiated:
- The patient or a family member
can call a local Home Health agency and request
an evaluation. (This is the best way to determine
if the patient meets the criteria for the program.)
- The patient's physician can
call in the referral (or tell the patient/family
to call it in).
Who pays for home health?
Most insurance companies cover Home Health. This
benefit can be accessed through the case manager
or by the physician
How long can a patient
be on home health?
The patient is evaluated every sixty days. As long
as the patient continues to meet the criteria they
can continue to access the program.
Can I keep my own doctor?
Your primary care physician will continue to monitor
the care and provide guidance to the Home Health
team. If they are unwilling or unable to do that,
Haven HealthCare™ can and will provide physician
services.
Can we hear about the
program before we discuss it with the MD?
Any patient can receive an evaluation for services
without a physician's order. The HealthCare™
staff, however, cannot provide any care or service
without a doctor's order.
What if I have to go to the
hospital?
The home health services can be resumed, upon return
home, as long as there is a physician's order to
do so.
